Previously, the subtitle generation was hardcoded, making it unsuitable
if you didn't want all of the displayed values. This has been replaced
by a format string that is configurable in the settings, using predefined
tags that are replaced with the values. The default value for this has
been set to (mostly) match the currently generated subtitle string. This
also provides a good starting point for users that want to modify the string.

This is a messy commit, because the "qPref" system relies
heavily on QString, which means lots of conversions between
the two worlds. Ultimately, I plan to base the preferences
system on std::string and only convert to QString when
pushing through Qt's property system or when writing into
Qt's settings.

Remove the preference settings dealing with thumbnails (currently under
General preferences and Profile preferences) and put them in a newly-created
Media preference tab.
